In order to qualify for these Limited Warranties, you must have this puppy examined by a licensed veterinarian within five working days of the sale.


If within five working days a veterinarian determines that your puppy is a poor health risk, you may return your puppy to Lakeside Pets and receive a full refund. A statement by the veterinarian must be in writing and available to the store at the time of the return. Any puppy that is returned will be under the care of the store veterinarian.


If within the first 21 days an illness does occur, you must bring this puppy to Causeway Animal Hospital for treatment at no cost to you. The following illnesses are specifically included: Respiratory illness, ear infections, eye infections, eyelid disorders, hernias, demodectic mange, sarcoptic mange, skin infections, cherry eye, stenotic nares, cryptorchidism, parasitism, gastrointestinal disease, infectious diseases including distemper and parvovirus and kennel cough, and hypoglycemia.


The following conditions will not be covered under this warranty: Cosmetic conditions, routine health care, injury, accidents, and behavioral problems.


For purchasers that live 60 miles or more away from Metairie, or in cases where treatment is referred to another veterinarian by Causeway Animal Hospital, Lakeside Pets will pay veterinarian fees for necessary treatment of illness (as stipulated above) for 21 days in the amount of 70% of reasonable veterinarian fees, limited to and not to exceed the purchase price of the puppy.
A claim under this provision must include an itemized statement that identifies your pet and describes the diagnosis of covered illness. This statement must be submitted within 21 days of rendered services and must be signed by the attending veterinarian, or the claim will be denied.


Purchaser assumes liability for all non-covered expenses.


If your puppy should die due to illness (accidents and injuries are not included) within one year of purchase, Lakeside Pets, Inc. will issue a credit in the amount of your original purchase price that you may use towards the purchase of another puppy. In order to make a claim under this provision, your puppy must have been under the care of a licensed veterinarian, which would include a complete immunization and worming program.
A claim under this provision must include a veterinarian statement as to the cause and date of death.

GENERAL TERMS AND CONDITIONS


If you are eligible for a refund under this warranty, Lakeside Pets, Inc. will issue a company check within 14 days after the receipt of all documentation substaining your claim.


Lakeside Pets, Inc. does not warranty that you will be able to use your puppy for showing or breeding purposes.


Lakeside Pets, Inc. does not warranty the adult size of your puppy.


Lakeside Pets, Inc. does not warranty your compatibility with your puppy. If within 48 hours of purchase, for any reason, you cannot keep your puppy, you may return the puppy to Lakeside Pets, Inc. for a refund, less a $150.00 service fee.
